The illicit drug trade is adapting its products, containers and pathways at a faster pace than governments are changing their response. Operation Lionfish-Mayag IV deserves recognition. And it shows the risks of mistaking spectacular seizures for durable defense.
Led by INTERPOL from 18 May to 4 June 2026, the operation resulted in a record 7,247 seizures of drug and pharmaceutical products totaling over $1.1 billion (650 tonnes). However, around 94% of the seized volume was made up by cannabis and cannabinoids. To describe this as a haul of 650 tonnes of synthetic-drugs would be misleading.
A much sharper warning comes from the finding that 17 tonnes of ketamine for the first time eclipsed methamphetamine in INTERPOL’s largest synthetic-drug operation by volume. Indonesia seized 1,975 litres of etomidate while authorities also intercepted another delivery and held back 724 kilograms of liquid DMT bound for the vapes. Mauritius discovered an operating fentanyl laboratory; China seized 307 kg of cocaine from a previously unknown maritime route between South America.
Collectively these results hint on a diversified market regarding its substances, delivery methods and sites. What remains is that the use of a vape can serve as a covert delivery method for a drug. The diversion of the drug are possible because because legitimate pharmaceutical supply chain. A line of shipping connects producers with a market that gets cultivated recently.
How Social Institutions Will Respond That Adaptability Is the Question

Pharmaceutical and chemical companies should take care to verify individuals, monitor irregular orders, and report possible signs of diversion. It is only with transparent reporting that regulators can deal appropriately, via criminal sanctions for wilful facilitation and other civil penalties for clear compliance failures whilst not undermining proper medical access. Patients who need anesthesia should never be collateral damage in the war on illegal markets
The exchange of coordinated risk analysis, rapid evidence preservation and responsible information sharing between postal operators, customs agencies and online marketplaces is imperative. Controls should review on suspicious shipment patterns and declarations. The suspension of all legal trade traps average businesses while tempting traffickers to move elsewhere.
Policing must be accompanied by public health. Broaden laboratory testing of suspicious vape liquids, provide specific alerts about found substances and train emergency clinicians to identify unknown exposures. Young people require credible information and private means to access care. Fear of punishment can turn a medical emergency into a mortal trap in waiting.
It is time for Indonesia, where the etomidate seizure originated and was most probably destined for distribution, to increase customs surveillance particularly at shipping centres; enhance cooperation between customs agents, drug-control authorities and health regulators; and implement mechanisms that quickly alert doctors when adulterated products enter circulation.
